How to get a temporary ID card

Follow the instructions below to get 2017 proof of coverage/temporary ID cards for medical/prescription plan, dental and vision coverage. Note: You can also download temporary ID cards using your carrier's mobile app.

Medical/prescription plan

 
If your 2017 plan is:Follow these steps:
Aetna PPO 800, Health Fund or Aetna Indemnity For proof of medical coverage:
  1. Go to aetna.com.
  2. Log in or create a username and password.
  3. Click Get an ID Card and follow the prompts.
  4. OR, click Coverage & Benefits, then select Order ID Cards or Print Temporary ID Cards.
  5. Temporary ID cards can also be requested by calling Aetna customer service at 1-800-522-6710.
For proof of prescription drug coverage: See Express Scripts information below.
UnitedHealthcare (UHC) PPO 800 or Health Fund For proof of medical coverage:
  1. Go to myuhc.com.
  2. Register on the website by creating a username and password, and then click Register Now.
  3. From the home page, click Print an ID Card.
  4. Click Print in the box titled Print Medical ID and Pharmacy ID Cards.
  5. Temporary ID cards can also be requested by calling UHC customer service at 1-866-480-4989.

Express Scripts prescription drugs (for participants enrolled in Aetna or UnitedHealthcare medical/prescription plans listed above) For proof of prescription drug coverage: You can use your Social Security number and Dell group number DLLAto pick up a prescription at a local pharmacy. You can also:
  • Print a temporary or replacement ID card online at express-scripts.com. Log on, go to My Prescription Plan and select Get Member ID Cards.
  • Request replacement ID cards by calling Express Scripts at 1-866-272-6695.
  • Your ID card is also available on Express Scripts' mobile app.
Kaiser Permanente POS (Hawaii only) Until you receive your ID card, you can provide a state-issued photo ID to your provider when you receive service, or your provider can verify your coverage by contacting Kaiser at 1-808-432-5955 and providing Dell group number 03803.
Kaiser Permanente DHMO (California only) Northern CA: Until you receive your ID card, you can provide a state-issued photo ID to your provider when you receive service, or your provider can verify your coverage by contacting Kaiser at 1-800-464-4000 and providing Dell group number 603936.
Southern CA: Until you receive your ID card, you can provide a state-issued photo ID to your provider when you receive service, or your provider can verify your coverage by contacting Kaiser at 1-800-464-4000 and providing Dell group number 231418.
Harvard Pilgrim (Maine, Massachusetts, New Hampshire only) Until you receive your ID card, your provider can verify your coverage by contacting Harvard Pilgrim at 1-888-333-4742 and providing Dell group number 055651.
 

Dental

 
If your 2017 plan is: Follow these steps:
Delta Dental You will not receive an ID card, but you can give your dental provider your Dell badge number, instead of your SSN, to confirm your coverage with Delta Dental. Your provider should contact Delta Dental directly at 1-800-693-2659 to verify coverage. The Dell group number is 17863.
 

Vision

 
If your 2017 plan is: Follow these steps:
Vision Service Plan (VSP) You will not receive an ID card. Your provider should contact VSP directly at 1-800-877-7195 to verify coverage. The Dell group number is 01109172.
